Dating Pregnancy: What Is the Most Accurate Method?

In the second trimester, this may be extrapolated to an estimate of gestational age and an estimated due date EDD. If the available ultrasound equipment allows for measurement of an ellipse, this may be drawn around the outside of the calvarium. Intrauterine Growth Restriction: Identification and Management

Metrics details. Maternal gestational diabetes GDM is an established risk factor for large size at birth, but its influence on intrauterine fetal growth in different ethnic populations is less well understood. Here, we examine the joint associations of GDM and ethnicity with longitudinal fetal growth in South Asian and White European origin women. This study included 10, singletons White European and South Asian from a prospective cohort of women attending an antenatal clinic in Bradford, in the North of England.

Ultrasound measurements of fetal head circumference HC , femur length FL abdominal circumference AC , and estimated fetal weight EFW , and corresponding anthropometric measurements at birth were used to derive fetal growth trajectories. Associations of GDM and ethnicity with these trajectories were assessed using multilevel fractional polynomial models.

The role of ultrasonography in obstetric practice has continuously evolved since its introduction more than 40 years ago. Indeed, it is difficult to imagine a modern obstetric practice without access to the information provided by real-time ultrasound. Using this imaging technique, clinicians can detect pregnancy as early as 3 weeks after conception, confirm or revise gestational age with reasonable accuracy, diagnose multiple gestation in early pregnancy, confidently diagnose fetal death at any gestational age, assess fetal well-being, evaluate amniotic fluid volume, and diagnose a broad variety of fetal malformations.

Advances in sonographic imaging in the past decade are attributable to both improved image resolution and to better clinical interpretation of ultrasound findings. It is reassuring that in human pregnancy no adverse bioeffects appear to be caused by diagnostic ultrasound.

Estimating gestational age from ultrasound fetal biometrics. Obstet Gynecol ; When dating pregnancies, the clinician deals with two sources of information: patient history and ultrasound findings.

The latter is certainly the most objective source, but there are inherent machine- and human-related drawbacks that affect the accuracy of these ultrasound dating methods. The study involved non-Hispanic black

In this study, the Hadlock models for fetal dating using single and multiple biparietal diameter (BPD), head circumference (HC), abdominal graphic scan.

The main goal was to investigate the relationship between prenatal sonographic parameters and birth weight in predicting neonatal body composition. Standard fetal biometry and soft tissue parameters were assessed prospectively in third-trimester pregnancies using three-dimensional ultrasonography. This parameter alone explained Birth weight similarly explained AC and estimated fetal weight EFW accounted for only Skeletal growth parameters, such as FDL Fetal growth is a result of complex interactions between several maternal, fetal and placental mechanisms.

A final classification of neonatal growth outcome depends on how this development is defined. Over four decades ago, Battaglia and Lubchenco 1 developed a landmark classification for neonatal growth outcome and this statistical approach has been applied to fetal weight assessment as well. Unfortunately, this approach does not distinguish between fetuses that are small or large, but otherwise normal, from others that are truly malnourished.

Soft tissue assessment may provide additional information about generalized fetal nutritional status. Several related parameters, such as mid-arm fat and lean mass, mid-thigh fat and lean mass, abdominal fat mass, subscapular fat mass, cheek-to-cheek diameter and buttocks have been used to assess fetal body composition.

The 18+0 to 20+6 week fetal anomaly ultrasound scan. 15 practicalities of a solution to combining dating and screening requirements at the early If the EDD was not previously assigned, the pregnancy should be dated by HC or FL.

Biparietal diameter BPD is one of the basic biometric parameters used to assess fetal size. In the second trimester this may be extrapolated to an estimate of gestational age and an estimated due date EDD. The BPD should be measured on an axial plane that traverses the thalami, and cavum septum pellucidum. The transducer must be perpendicular to the central axis of the head, and thus the hemispheres and calvaria should appear symmetric.

BPD has been shown to be accurate in predicting gestational age from 14 to 20 weeks 2. The variability increases after this time. BPD may also be influenced by factors such as abnormalities of head shape, breech presentation, or multiple gestations. Head circumference HC may be a more reliable measurement if there is variant head shape 4. Please Note: You can also scroll through stacks with your mouse wheel or the keyboard arrow keys. Anatomy scan (mid-pregnancy ultrasound)

This type of ultrasound scan is referred to as a fetal growth scan. During the fetal growth scan, various measurements are taken of the fetus. The measurements are plotted on a growth chart, according to the number of weeks pregnant that you are at the time of the scan gestational age. The main fetal measurements taken for a growth scan include:.

Although the determination of Estimated Fetal Weight (EFW) is not considered to be a days Based on: D U/S D LMP D IVF conception Last Scan Date: Number.

With the addition of CRL, R2 was 0. Complications of Pregnancy. The fetal foot is one of the first structures identifiable early in the human embryos. At the end of the fourth week of embryonic development, the limb buds appear as outpouchings from the ventrolateral body wall. The fingers and the toes are distinct and separated in the hands and feet [ 1 , 2 ]. This linear correlation has been confirmed by studies on live fetuses in utero on transabdominal [ 4 , 5 , 6 ] or transvaginal scans [ 7 ] or on dead fetuses at abortion [ 8 , 9 , 10 ] or stillbirth [ 11 , 12 ], and nomograms have been developed for assessment of fetal gestational age with foot length FT from the first trimester to later gestation.

Hence, fetal foot length could by itself stand as a proxy for gestational age even in early pregnancy. Conventionally, crown-rump length CRL is used as the reference parameter for assessment of fetal gestational age in the first trimester ultrasound scan [ 13 , 14 ]. The use of routine first trimester ultrasound scan has been shown to be associated with a reduction in induction of labor for post-term pregnancy [ 15 ].

However, there is little information on the comparison of other fetal parameters including biparietal diameter BPD , head circumference HC , abdominal circumference AC , femur length FL , and foot length to CRL in the assessment of gestational age in early gestation [ 16 ]. This information may be important for the assessment of fetal gestational age in the first trimester and subsequent management of pregnancy. Moreover, the ratio of some of these parameters will also be calculated and presented, as the availability of such ratios may be helpful when fetal abnormality is suspected on ultrasound examination in early pregnancy [ 17 , 18 , 19 ].

Only pregnancies with normal outcomes were included in the analysis and excluded if the entire foot could not be clearly seen during the ultrasound examination. The fetal foot length was measured from the most posterior point of the foot in its long axis to the tip of the first or the second toe whichever was longer Figure 1.

Also see Appendix 8 — Guidelines for the Midtrimester Obstetric Scan This scan is available for all patients as part of routine obstetric management. Cochrane evidence indicates a reduction in induction for post-dates pregnancy, increased rates of termination for fetal malformation and increased diagnosis of multiple pregnancy. There is no current evidence that perinatal mortality rates are reduced by this investigation. It is not recommended as a screening test for aneuploidy. Diagnostic limitations of the examination need to be discussed with the patient.

Technical limitations which may reduce diagnostic accuracy at the time of the examination should be recorded e. A morphology scan may also be performed as a second opinion for confirmation or further investigation of pathology. The Ultrasound Examination The ultrasound examination should include: Fetal number; Fetal heart activity; Fetal biometry including BPD, HC, AC and FL; Anatomical survey see Appendix 8 – Guidelines for the mid-trimester obstetric scan ; Fetal movement; Amniotic fluid assessment; Cervical length; Placental site, distance of lower margin from internal os; and Maternal anatomy including uterus and adnexal masses.

Ultrasound estimation of gestation should be used in preference to menstrual dates if the discrepancy is 10 days and there is no previous dating scan or known conception from artificial reproductive technologies. Acknowledgement should be made that the dates have been changed; Fetal anatomical survey should be reported and commented on with respect to detection of malformations and quality of assessment. Examinations that are limited or suboptimal because of fetal position, maternal obesity or scars should be acknowledged.

A judgment should be made as to whether a repeat examination would improve diagnostic accuracy; Fetal abnormalities should be reported to the referring doctor on the same day to expedite management choice for the pregnant woman. Interpretation of the clinical significance of these is controversial.

My baby’s age on the scan report is different to my stage of pregnancy. What does this mean?

A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures. Dating scans are usually recommended if there is doubt about the validity of the last menstrual period.

By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures.
